What is Metoidioplasty?

Similar to a phalloplasty, metoidioplasty is a female to male sex reassignment surgery designed to construct a penis. Metoidioplasty uses testosterone replacement therapy to enlarge the clitoris about 4-5cm to create a neophallus. The testosterone-induced growth of the clitoris lengthens and straightens the neophallus; metoidioplasty is possible because the clitoris and penis are developmentally homologous.

How is Metoidioplasty Performed?

Dr. Schechter performs metoidioplasty ranging from simple release to urethral lengthening. The surgeon separates the enlarged clitoris from the labia minora, and cuts the suspensory ligament in order to lower it to the correct position of the penis. The labia minora can be used to protect the graft and provide greater girth. If performed with urethral lengthening, the goal of metoidioplasty is the ability to urinate while standing. Generally, metoidioplasty does not allow penetrative intercourse, but can be combined with a vaginectomy and scrotoplasty. Typically, testicular implants (scrotal reconstruction) are placed at a second stage. A catheter will be placed in the urethra and typically remains in place for approximately 2 weeks. On occasion, a suprapubic catheter may be utilized.

What are the Advantages of Metoidioplasty?

Metoidioplasty is less complicated than phalloplasty (requires shorter operating and recovery time) and are usually more affordable. Other pros include a more natural, natal-appearing penis as well as erotic sensation, unassisted erections, and very little scarring.

What are the Disadvantages of Metoidioplasty?

Metoidioplasty patients are far less likely to be capable of sexual penetration after they recover from surgery. Additionally, metoidioplasty results in a small sized penis, while phalloplasty yields more enlargement.
